    You may also submit a contact form and our office will follow up, or schedule a complimentary discovery call at your convenience.

  • No. Illinois is a direct access state, which means you can begin physical therapy without a physician referral. Under Illinois law, a physical therapist may evaluate and treat patients without a script.
    However, some insurance plans may require prior authorization or a referral if you plan to submit for out-of-network reimbursement.

  • We are located in Northwest Chicago in the Norwood Park neighborhood, near Park Ridge, IL.

  • Hours: Monday–Thursday 9:00 AM–6:00 PM
    Friday 8:00 AM–4:00 PM
    We are closed on weekends and holidays.

  • Yes. We have plenty of free street parking right outside the building, as well as a parking lot directly across from our entrance.

  • We are out-of-network with all insurances. This model allows us to provide high-quality, personalized care that isn’t dictated by insurance limitations. To learn more about our clinic model, visit our About page.

  • Yes. We accept HSA and FSA cards for physical therapy services.

  • We collect payment at the time of service. Upon request, we provide an itemized receipt (superbill) for you to submit to your insurance for possible out-of-network reimbursement.
    Please note: Medicare and Medicaid do not reimburse for out-of-network physical therapy services. Because we cannot opt out of these plans, patients with Medicare or Medicaid are not eligible for reimbursement. To learn more check our our Payment and Reimbursement page.
